Most people researching hormone replacement therapy already know something is off. The question they are really asking is what actually happens after they call a local HRT clinic. The process is more straightforward than most people expect, and nothing about it is rushed.
The First Consultation
The starting point is a consultation with a provider in our network. You can do this in person at our Denville location at 161 East Main Street, or via telehealth if that fits your schedule better.
Your provider reviews your symptoms, health history, current medications, and treatment goals. This conversation shapes which labs are ordered and how results are interpreted later. Nothing is prescribed at this stage; the consultation is diagnostic, not prescriptive.
Lab Work: What Gets Tested and Why
After your consultation, lab work is ordered based on your symptoms and profile.
For men, testing typically includes total testosterone, free testosterone, estradiol, LH, FSH, PSA, CBC, and a metabolic panel. For women, the panel commonly measures estrogen, progesterone, testosterone, FSH, and thyroid markers. If peptide therapy is part of the conversation, growth hormone markers may be added.
A result that falls within the broad “normal” reference range may still be low for your age and symptom profile. Our provider network evaluates lab results in the context of your symptoms, not as a standalone number.
How Your Protocol Is Built
Once lab results are reviewed, your provider creates a treatment plan specific to you. This covers which hormones are being addressed, the delivery method that fits your lifestyle and clinical profile, starting dose, and when follow-up labs are scheduled.
For men starting testosterone replacement therapy, the protocol may also include anastrozole to manage estrogen conversion, or gonadorelin for patients who want to preserve natural hormone signaling. For women starting bioidentical HRT, the protocol accounts for where they are in the perimenopause-to-post-menopause transition.
Delivery Options
We work with multiple delivery formats so the method fits your life, not the other way around.
For men, testosterone is available as subcutaneous or intramuscular injections, topical gels, compounded creams, or implantable pellets. For women, estrogen and progesterone are available as creams, gels, patches, and compounded preparations, including Biest cream, a combination of estriol and estradiol in proportions individualized to the patient. Compounded formulations are available across both service lines for patients who need dosing precision that commercial products cannot provide.
Monitoring After You Start
Treatment does not end when the prescription is written. Follow-up is built into the protocol from the start.
Most patients have a check-in at the 3-month mark, followed by a 6-month evaluation and annual comprehensive reviews. At each follow-up, labs are re-drawn, levels are confirmed in the therapeutic range, and dosing is adjusted if needed. What works at the starting dose does not always stay optimal as your body responds, and adjustments based on actual follow-up data are part of how the protocol stays effective.
What Patients Commonly Report
Energy and mood typically show the earliest changes. Many patients notice a difference within the first 4 to 6 weeks.
Sexual function, body composition, and sleep tend to improve over a longer arc, generally 3 to 6 months of consistent treatment. Individual results vary based on starting hormone levels, protocol adherence, and overall health. Our provider network does not make outcome guarantees; what we do is establish a protocol based on your confirmed labs, monitor your response, and adjust as needed.
