How Your Protocol Is Built

Once lab results are reviewed, your provider creates a treatment plan specific to you. This covers which hormones are being addressed, the delivery method that fits your lifestyle and clinical profile, starting dose, and when follow-up labs are scheduled.

For men starting testosterone replacement therapy, the protocol may also include anastrozole to manage estrogen conversion, or gonadorelin for patients who want to preserve natural hormone signaling. For women starting bioidentical HRT, the protocol accounts for where they are in the perimenopause-to-post-menopause transition.

 

Delivery Options

For men, testosterone is available as subcutaneous or intramuscular injections, topical gels, compounded creams, or implantable pellets. For women, estrogen and progesterone are available as creams, gels, patches, and compounded preparations, including Biest cream, a combination of estriol and estradiol in proportions individualized to the patient. Monitoring After You Start

Treatment does not end when the prescription is written. Follow-up is built into the protocol from the start.

Most patients have a check-in at the 3-month mark, followed by a 6-month evaluation and annual comprehensive reviews. At each follow-up, labs are re-drawn, levels are confirmed in the therapeutic range, and dosing is adjusted if needed. What works at the starting dose does not always stay optimal as your body responds, and adjustments based on actual follow-up data are part of how the protocol stays effective.

Energy and mood typically show the earliest changes. Many patients notice a difference within the first 4 to 6 weeks.

Sexual function, body composition, and sleep tend to improve over a longer arc, generally 3 to 6 months of consistent treatment.
